Medical and Social Services

The Medical House Call Program offers innovative medical and social services in the community and the hospital. Our main medical services are primary care in the home and an inpatient geriatrics hospital unit. Our geriatricians take care of the same elders in the home and in the hospital, to promote continuous and safe care. Our doctors and nurse practitioners carry a modern version of the "doctor's black bag," and use state-of-the-art portable technology in the home. Our social workers connect patients and caregivers to community services, such as home health aides, that support living at home. On house calls, our staff members can see how patients actually live and develop a plan of care for their special circumstances. During weekly team meetings, staff members from the Visiting Nurses Association, the New Hampshire pharmacy, and Community Hospice join our clinicians to discuss the plan of care.

Medical Services

  • House calls by doctors and nurse practitioners for primary and urgent care on a monthly and as needed basis
  • Diagnostic tests at home (blood draws, X-rays, oxygen monitoring)
  • Home delivery of medications and equipment
  • Coordination of home nursing, physical therapy and hospice
  • On-call physician: 24 hours a day, seven days a week by phone
  • Hospital care by House Call doctors on a Geriatrics Inpatient Unit at the Washington Hospital Center
  • Coordination of specialty care and procedures at Washington Hospital Center

  • Coordination of in-home resources for meals, home health aides, respite care and financial assistance
  • Case management for the Elderly and Persons with Disabilities Medicaid Waiver
