Post-Transplant Care

After discharge, the patient is initially seen in the transplant clinic at frequent intervals to monitor renal/pancreas function and levels of immunosuppressive medications. As convalescence progresses, patients are seen alternately with their nephrologist. Laboratory data and transplant status are determined in real time and appropriate same-day adjustments are prescribed. Our nurses monitor the patient's vital signs, labs, X-rays, medications and changes in emotional well being to create a regimen of individualized care. The goal is to return patients to a normal state of physical and emotional health as quickly as possible. The transplant clinic features doctor led care and provides the services of two full-time RN's and one full-time NP. Our hours are 8:00 a.m. - 4:30 p.m., Monday-Friday.
