Nursing Care Delivery Model

Health care environments are increasingly complex for both patients and health care professionals alike. To meet these challenges, the nursing care delivery model at MedStar Washington Hospital Center is designed to facilitate a seamless patient care experience across all continuums of care, from admission through discharge. The model of care organizes nurses/care providers into teams responsible for a specific 12-15 patient cohort. The group is assigned geographically to optimize the nurses' time with their patients. Each group has one Clinical Care Facilitator (CCF), who works with the assigned RNs and Patient Care Technicians for the patients. The CCF is a clinical nurse leader with a BSN and acute care experience, who provides leadership for the patients and the health care team. The CCF serves in the role of lateral integrator, ensuring that the interdisciplinary services achieve the highest patient outcomes and an exceptional experience for every patient. The CCFs act as clinical mentors, and provide clinical education at the point of care.
